MAJOR RESPONSIBILITIES:

• Physical Therapist to provide physical therapy to students in the elementary & middle school levels in a school setting.
• Responsible for NYS Medicaid required paperwork.

Job Qualifications

REQUIRED MINIMUM QUALIFICATIONS:

• NYS licensed and currently registered as a Physical Therapist
• Must have own transportation
